Calvert Hospice was recently recognized as achieving a Level One Partner rating by the National Hospice and Palliative Care Organization. Calvert Hospice is working with area Veterans to improve their access to hospice and palliative care and how they transition from one care setting to another. “We want to ensure that every Veteran who requests hospice care is able to receive hospice care at the time and place in Calvert County of his or her choosing. We do this by initiating a comprehensive end-of-life plan designed especially for our Veterans,” stated Brenda Laughhunn, Executive Director of Calvert Hospice.
More than 1,800 Veterans die every day in this country and the vast majority of them die in the community; this means that every hospice is caring for Veterans. According to the 2010 Census, there are 8,986 Veterans in Calvert County which is more than 10% of the total population of 88,737.
Hospice-Veteran Partnerships (HVPs) are coalitions of community hospices, Department of Veterans Affairs (VA) facilities, end-of-life care and community organizations, and others working together to ensure that excellent care at the end of life is available for our nation’s Veterans and that support is available for their families. The national HVP program is a joint effort of the VA Hospice and Palliative Care Program and the National Hospice and Palliative Care Organization.
Calvert Hospice collaborates with a variety of organizations to improve end-of-life care for Veterans by sharing information about the services they provide and how Veterans can access them, and how to resolve issues related to referral and reimbursement.
